The information we collect from you is necessary to enable us to provide you with the appropriate clinical services and to manage our clinics. This information will include your contact details, medical history, health care provider and insurance details.

We will need to collect information from you when you initially contact us to make an appointment. When you attend our clinics for the first time, we require you to complete a New Patient Details and Consent Form which includes patient privacy consent, so we can collect the necessary details.

We may need to obtain further information from you during the course of providing our services, to ensure we can provide the best possible care.

We take all reasonable steps to ensure your information is accurate, complete and up to date and form time to time we will confirm all your details are correct. Please let our staff know if the personal information we hold about you is out of date or incomplete.


Use and Disclosure of Information

Personal patient information that we collect will be used for assessing, diagnosing or treating a patient's healthcare needs. We will also need to use some information for administrative and billing purposes, including Medicare and Health Insurance purposes.

Our staff will use and disclose your information only to provide you with the highest quality healthcare services and to report back to your referring doctor. This may include disclosure to other doctors and medical staff to assist in your care or for teaching purposes, or in emergency situations where access is required for treatment purposes.

We will only use your personal information for the purposes for which you have given us the information, unless it is necessary in an emergency situation or is required of us under law.


Access to Records

We will provide your results and reports directly to your referring doctor, so you can receive the results where treatment (if required) can be discussed and managed. Your doctor will be able to provide you with a copy of your results.

If you require direct access to your records, please provide proof of identification and submit your request in writing. Patients may be charged a fee for the administrative costs to retrieve and provide copies of your records.


Security of Information

Clinical Integration Services takes all reasonable steps to protect your personal information. Your personal information may be held in various forms including electronic records and images.

All personal information is held securely, whether in highly protected information systems or in hard copy format in a secured environment. Our staff and consultants always act in the patient's best interests when providing healthcare and testing services and we ensure there is no conflict of interest that affects the quality of the services we provide.
